Disclaimer and Terms and Conditions of Enrollment
MyCare Plan™ (also known as the UWE Will and Trust) will provide to enrollee the following: Last Will, Living Trust, Financial Power of Attorney, and Health Care Power of Attorney. Once you have been enrolled, you will be responsible to complete a data input document that will be used to create your will and trust documents. There is no time limit on completing the data input document; however, the will and trust cannot be produced without the necessary information.
You may cancel this contract within three (3) business days after enrollment and receive a full refund. After three (3) business days, no refund will be issued. If paying by check, your check may be electronically deposited.
Once final approval on the input data has been given by you, any changes requested will be subject to a $149.00 fee.
You understand and authorize United Wealth Education (UWE) to charge you $39 per month, unless you notify UWE of your intent to cancel your enrollment in the MyCare Plan. Payments will be automatically debited from the payment method provided - credit or debit card or checking account. An email reminder will be sent each month prior to your billing due date. Failure to make your payment on the established due date will result in suspension/cancellation of all activated programs.

By using our document preparation service, no attorney client relationship will be created with UWE Further, e mail or written correspondence with UWE will not be covered by any attorney client privilege.

Our document preparation service is not designed for people with large or complex estates, and will not meet the needs of everyone. Neither the services we provide nor the documents we prepare take into account or make any provision for SSI or SSD benefits, Medicaid planning or eligibility, estate or inheritance taxes, estate or inheritance tax planning, fraudulent transfers, or the effect that putting any assets into trust may have on creditor claims. Our services are not designed for people with total assets in excess of the federal estate tax threshold amount ($5,000,000 in 2011 and adjusted for inflation thereafter).

Documents prepared by UWE are not guaranteed to comply with any applicable state laws. The services of UWE are not valid outside of the United States and any offer to perform such services is void where prohibited by law.
If a dispute arises out of this contract, and if it cannot be settled by the parties, the parties agree first to try to settle the dispute by mediation administered by any mutually agreed upon mediator pursuant to the Commercial Mediation Procedures of the American Arbitration Association ("AAA"). If they do not resolve the dispute within 60 days, then, upon notice by either party to the other, all disputes, claims, questions or differences shall be finally settled by binding unappealable arbitration administered by any mutually agreed upon arbitrator (or if none can be agreed upon, then by the AAA) pursuant to the Commercial Arbitration Rules of the AAA. In any such proceeding, which must take place in Oakland County, the State of Michigan, each party shall bear their own attorney fees and costs, however, the fees and costs of any neutral mediator or neutral arbitrator shall be paid by UWE. Additionally, you agree not to initiate or participate in any class action proceeding against UWE, whether in a judicial or mediation or arbitration proceeding, and you waive all rights to become a member of any certified class in any lawsuit or proceeding.
These Terms and Conditions run in favor of UWE, its affiliates, agents, licensees, contractors, officers, directors and employees.
